1. Download the right gmapsup.img file http://wiki.openstreetmap.org/wiki/OSM_Map_On_Garmin/Download
2. Connect your GPS and put it in mass storage mode http://wiki.openstreetmap.org/wiki/OSM_Map_On_Garmin/Mass_Storage_Mode
3. Rename/move the exsisting gmapsup.img file on the device as backup
4. Copy the new gmapsup.img file to your device where your old file was before you renamed it

The file layout on the device is just like before you started, but the gmapsup.img file contains the osm map. The name of the file is important.

As Gnonthgol's comment, but it's gmapsupp.img instead of gmapsup.img.

There's always the easy way: Get yourself a premade gmapsupp.img from http://garmin.na1400.info/routable.php

But an easy start with mkgmap is needed too.
My first try is not visible on my Garmin.
I did:
1. download a small area with josm
2. save it as data.osm
3. java -ea -jar ..\mkgmap-r1512\mkgmap.jar --description='Openstreetmap Test' --gmapsupp data.osm
4. copy /Y gmapsupp.img g:\Garmin
Any clue what is wrong?

I had similar problems when I started some time ago;

I take no credit for this but hope it helps.

Cloudmade maps are another option

Backup the original gmapsupp.img file ( or simply rename it to eg gmapsup_orig.img)

download the gmapsupp.img files for which ever country you need. Copy to the SD card to the folder 'garmin', if it doesn't exist create it
Cloudmade maps from OSM for nearly every country are available here
