As for ref=Th 38, I've simply used the ref of the continuation of the road to the west. The road there is using ref=Th 38.

As to moving int_ref=AH11 to the north: the routing of AH11 is along the northern way, and not the southern one. There are highway signs on the northern way, not the southern one.
