Thanks for pointing it out, turn lane fixed and placement tag added.

I've opened a fixme (4889411) for the toll tag issue.

There is no "left turn" at the intersection to Chapel Rd, the left lane continue to Chapel Rd. They are both straight on (see the solid line marking), The relation maybe not necessarily as I created turn lanes later on.

Thanks to your comment, now I have added the fence and gate to the road and added access=permit.
