Thanks for your input, actually what happened was that during the upload an error occurred then I re-upload the data later, which might be the cause the of issues, but I will try to avoid this issue in future.

I have fixed the issues, thanks for reaching out to me. I always use Josm validator but some how I missed this during upload.
