sure it could, or at least be complemented with the destination tag. The page you linked applies to the italian routes network managed by the CAI association. This changeset is in Switzerland. Nothing forbids the use of destination also in Switzerland, though.
Personally, I'm rather a fan of the destination_sign relation, as it can provide more details and sort out lack of clarity (example: when the same guidepost points to different routes leading to the same destination). This is what I mainly use nowadays. It can coexist with the destination tag.
In the old days I had used the description tag extensively, because of lack of established tags.
I agree that is should be replaced (or integrated) with either or both a destination or destination_sign tagging.
That's what I'm currently doing when I bump into those old description tags of mine.
It should be done for all of them. Problem is that I have a huge backlog, and fresh informations to be added as well which I tend to prioritize. I can't promise to update all of them anytime soon, and I would certainly appreciate any help by willing mappers there.

Yes. In this cas simply remove te description (you already added destination_sign relations).

Sorry, I can't. I have a huge mapping of node networks, hiking path, guideposts and snowshoe trails, especially in Jura (France)
We have an "OSM outdoor" Telegram group to discuss mapping of hiking, riding, mtb, skiing… if you are interested.
